What is ED?

Erectile Dysfunction (ED) is a very prevalent men’s sexual health condition. It impacts as many as half of all men 40-years-old and up. ED occurs when a man is unable to reach a state of erection or maintain it during intercourse. The condition itself is not considered dangerous; however, what could be causing ED in a man can be more serious, even life-threatening. These issues can include: a heart condition, systematic vascular disease, cancer, and others. Mental health issues can also arise from ED or contribute to it, like depression, low self-esteem, and anger.

Who does ED impact?

ED can impact men of all ages; however, it is most common in men 40 and older. Male arousal incorporates several internal systems and external stimulants that must work together in order for an erection to be possible. These include: the nervous and cardiovascular systems, brain, and hormones. What many men don’t like to admit or hear is that emotions play a serious role as well. Treatments and Side Effects

Treatments include:

  • Oral/Sublingual Medications: Work by increasing blood flow to the penis enabling it to expand and become hard. It causes an erection when a man is sexually aroused. Examples: Sildenafil, Vardenafil, Tadalafil, and Avenafil. – Potential Side Effects
  • Vacuum Therapy: Cylinder mechanism withdraws air creating a vacuum and enhancing blood flow to the penis. Then, user must place a rubber ring at root of penis to keep the erection by trapping the blood. – Potential Side Effects
  • Injection Therapy: Utilizes muscle relaxation to relax the penis and allow the flow of blood into the tissues causing an erection. Patient uses small needle to inject the muscle relaxant. Example injections include: Tri-Mix, Caverject, Alprostadiol, Papaverine, Phentolamine. – Potential Side Effects
  • Penile Implants: Surgical procedure implants inflatable balloons, a pump, and a reservoir into the penis, scrotum, and near the bladder. Inflating the pump causes reservoir fluids to fill the balloons and cause an erection. We refer to an outside vendor for this. – Potential Side Effects
  • Intraurethral Suppositories: A small, medicated pellet is inserted into the tip of the penis. It relaxes the blood vessels enabling blood to flow into the penis to help cause an erection. MUSE is an example provider for this treatment. We refer to an outside vendor. – Potential Side Effects

Please note: The treatments listed above have potential side effects. Click on each treatment to find out more about it and to see a list of potential side effects. Each individual patient’s results and side effects may vary with each treatment.

Oral/Sublingual MedicationsSildenafil (Viagra), Vardenafil (Levitra), Tadalafil (Cialis), and Avenafil: Oral or sublingual medications work directly on the blood vessels, causing the arteries to the penis to expand. It can cause an erection when the man is sexually aroused.
Potential Side Effects: Headaches, facial flushing, backache, congestion, upset stomach, bluish tinge to vision or abnormal vision, hearing loss, and prolonged erections (priapism). These drugs cannot be taken with nitrates due to the possibility of decrease blood pressure. Caution should be used with Alpha blockers for prostate enlargement and/or high blood pressure due to the possibility of decreased blood pressure.
Vacuum Therapy(a cylinder placed over the penis): By withdrawing air, a vacuum is created, mechanically enhancing the flow of blood into the penis. A rubber ring is placed at the base of the erect penis to trap blood and maintain the erection.
Potential Side Effects: Bruising, skin irritation and abrasions, discoloring of the penis, thickening of skin, vascular damage, pain, and diminished ejaculation.
Injection TherapyTri-Mix, Caverject, Alprostadiol, Papaverine, Phentolamine: During this process, the patient uses a small needle to inject a powerful muscle relaxant into the base of the penis. The relaxation of the muscle tissue allows blood to flow into the erectile tissues in the penis, which can then create an erection.
Potential Side Effects: Pain, bruising and tenderness, scarring, bleeding, abnormal liver enzymes, and prolonged erection (priapism).
Penile Implants(requires a surgical intervention): Two inflatable balloons are implanted in the penis, as well as a pump into the scrotum and a reservoir near the bladder. When the pump is inflated, fluid from the reservoir flows into the inflatable balloons to help create an erection. (this treatment option available via referral to outside provider)
Potential Side Effects: Pain, inflammation, malfunction.
Intraurethral Suppositories (MUSE): The patient uses an applicator to insert a small pellet of medication into the opening of the penis. The medication causes blood vessels to relax, so the penis fills with blood and becomes erect. (this treatment option available via referral to outside provider)
Potential Side Effects: Pain, burning sensation, and prolonged erection (priapism).

Low Testosterone

What is Low-T?

Low Testosterone is a medical condition in which a man displays a lower level of testosterone than is considered normal for his age bracket, height, weight, and other factors. A man who suffers with Low-T will oftentimes exhibit negative symptoms/health issues in more places than just in bed. Testosterone replacement:  Worsening of prostate issues (BPH and prostate cancer) including increased risk of developing prostate cancer. Changes in red blood cell level, cholesterol or lipid levels, hormone levels, and PSA level.  It can also cause permanent hair growth on face, chest, buttocks, or other areas of the body; head hair loss; mood or psychological changes such as increase anger or aggressive behavior; breast growth similar in appearance to that found in women; testicular shrinkage or atrophy; decreased sperm count leading to infertility; acne. Premature Ejaculation

What is Premature Ejaculation?

Premature Ejaculation (PE) is a men’s sexual health condition where a man will ejaculate early, earlier than he wants, oftentimes shortly after penetration with very minimal sexual stimulation. PE will usually cause tension, anxiety, and stress in relationships due to the unfulfilling sexual experience it causes. Having one instance or multiple rare occasions of Premature Ejaculation is common among men. Most will experience PE at least once in their lifetime. When a man begins to experience recurring Premature Ejaculation, then he should start to seek treatment.

And, if the PE is caused by a physical condition, then treatment options may include wearing a condom or implementing distraction methods to somewhat decrease sensitivity and delay ejaculation.

There are a number of different possible treatments for Premature Ejaculation, including relaxation exercises, condom use, control techniques, gels or creams, and reducing alcohol use, among others.
